Absolutely an original item, not a reproduction.  This item is a "cover" in philatelic terms--a Confederate States of America item of "Postal History" to be more precise.
The letter IS present and you are purchasing both the postal portion and the original letter.
COMBO Confederate #6 & #7 - showing stark extremes in printing on a complete folded letter postmarked - NEW YEAR'S DAY "RICHMOND VA. JAN 1 1863."
Addressed to "Dr. [Francis] Stribling, Staunton, Virginia" and written by Mrs. James E. Goddin on December 31, 1862.  Content deals with "ascertain[ing] the state of my husband's mind" and offering commentary and the desire to know more about her husband's mental state while under Dr. Stribling's care.  A fantastic item with a lot going for it.
